CONSENT REGARDING DATA PROCESSING

1) I voluntarily give my consent to process my data (e-mail, country, education, year of birth and other) for the purpose of participating in the WikiBest service in compliance with the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation);

2) I acquired full data from organiser of the WikiBest concerning its identity as the controller and regarding contact details of the controller and its representative;

3) I was informed by the controller of the purpose and legal basis of processing of my data;

4) the controller provided me with contact details of the data protection officer;

5) I was informed of categories of recipients of my data;

6) I was informed of the period during which my data will be stored / criteria of determining the period during which my data will be stored;

7) I was informed of the right to require the controller to provide access to my data, their rectification, erasure or restriction of their processing or of the right to raise an objection against the processing as well as of the right to data portability;

8) I was informed of the right to withdraw the consent on processing my data at any moment, without affecting the lawfulness of processing performed under the consent before its withdrawal;

9) I was informed of the right to lodge a complaint with the supervisory authority;

10) I was informed that submission of my data is the requirement necessary to participate in the WikiBest and that I am not obliged to provide them but the failure to provide the data shall prevent my participation in the WikiBest;

11) I was informed whether in relation to my data, automated decision-making will take place, including profiling and - if applicable - of the rules related to undertaking such activities, as well as of the significance and envisioned consequences of such processing for me.
